Case Management Coordinator Compensation
Pay Rate : $34.13 / hour
Case Management Coordinator Highlights
Fully remote work-from-home position
Approximately
75% regional travel
for face-to-face member visits
Full-time : 40 hours per week
Monday – Friday | 8 : 00 AM – 5 : 00 PM EST
Training conducted remotely via
Microsoft Teams (4–6 weeks)
Candidates
must reside in Miami-Dade County, FL
(Doral or Coral Gables)
Bilingual Spanish / English REQUIRED (read, write, speak fluently)
Case Management Coordinator Responsibilities
Conduct
telephonic and face-to-face assessments
with Medicaid Long Term Care / Comprehensive Program members
Develop, implement, and monitor individualized care plans
Coordinate services including :
Prior authorizations
PCP and skilled provider communication
Medication reviews
Condition management education
Community resources and support services
Utilize
critical thinking, motivational interviewing, and negotiation skills
to drive member engagement and outcomes
Perform comprehensive data review and documentation using care management tools
Conduct multidisciplinary reviews to achieve optimal member outcomes
Identify and escalate quality-of-care concerns through established channels
Provide education, coaching, and support to empower members in healthcare decision-making
Travel to
member homes, Assisted Living Facilities, and Skilled Nursing Facilities
throughout the region
Case Management Coordinator Requirements
Bachelor’s degree required
(Social Work or related field)
This is NOT a nursing role — no RNs or LPNs
Case Management experience
required
Long Term Care experience
preferred
Fully bilingual in
English & Spanish
(verbal and written)
Proficiency in
Microsoft Office
(Excel, Outlook, Teams)
Strong written and verbal communication skills
Ability to multitask and adapt in a fast-paced environment
Highly organized, detail-oriented, and able to work independently
Verifiable High School Diploma or GED required
Preferred Qualifications
Experience with Medicaid or Long-Term Care populations
Field-based or home-visit case management experience
Knowledge of community resources within Miami-Dade County
