AuthoraCare Collective is currently seeking an RN On-call Weekend (Integrated Health Services, Home Health and Hospice) . This is a F ull-time position : Friday, Saturday and Sunday 10 : 00am - 8 : 30pm.
Job Summary :
Triages incoming calls for patients on Integrated Health Services. Assess and identify patient problems and respond appropriately with clinical intervention. Complete in-home clinical visits as applicable to assess and treat patient. Coordinate with appropriate resources to meet patient's after-hours needs. Work closely with members of the care teams to ensure quality communication and patient care. Schedule and add appropriate documentation to EMR with each patient interaction including obtaining physician orders. Complete assigned EMR workflow.
Establish and maintain therapeutic relationship with patient, including providing disease education, medication management, clinical care and end of life care. Will send communication to appropriate discipline to ensure continuity of care. This nurse will bring experience and passion in working in an environment that follows AuthoraCare core values of humanity, compassion, empowerment, and excellence as well as inclusion of all team members, and patients.
Requirements
Education / Qualifications :
- Graduate of an accredited School of Nursing.
- Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
- CPR Certification.
- One year of experience in an acute, hospice, home health setting, or long-term care setting. Two years of experience preferred.
- Bachelor's degree in nursing preferred.
- Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
