James River Hospice is actively seeking a compassionate and dedicated Hospice Baylor LPN to join our expanding team in Richmond, VA! As an LPN, you will provide intermittent hospice care and support to your assigned patients in their place of residence while collaborating with the interdisciplinary team to ensure the best possible care experience.Schedule : Friday, Saturday, Sunday, Monday (12-Hour Shifts, 8 AM - 9 PM)What's in it for you?

CPR recertification paid for by the companyPrimary Responsibilities for Licensed Practical Nurses :
Provide intermittent care and support to hospice patients in their place of residence.
Implement the patient's care plan in coordination with the interdisciplinary team.
Monitor and document patient symptoms and changes in condition.
Administer prescribed medications and treatments.
Provide education and emotional support to patients and their families.
Assist in coordinating care among the hospice team, ensuring the patient's needs are met.
Maintain open communication with the interdisciplinary team and report any necessary updates to the RN Case Manager.
Participate in on-call duties as defined by the on-call policy.
Perform other duties as assigned.Qualifications :
Must be a Licensed Practical Nurse (LPN) with a current license to practice in Virginia.
At least one year of hospice or related nursing experience is preferred.
Graduate of an accredited LPN program.
