Responsibilities :

• As a Registered Nurse, you may honor your patient's wishes and help them achieve their goals
• Allow patients to leave this world with dignity
• Responsible for acting as the intermediary between Snowline clinical staff and third party triage partne
• Schedules visits for nursing and HHAs for the weeken
• Manages triage needs throughout the day and dispatches to available staff for visit
• Acts as a resource to the rest of the clinical teams
• Ensures patient follow up needs are scheduled and addressed
• At times may need to make patient visits to support patient care needs
• In close collaboration with other disciplinary teams (including admissions), you will be actively supporting quality and timely care and assist in improving the lives of others
• Enjoy meaningful interactions with your patients and families
• Plan and organize the best hospice care to meet the needs of the patients and families within their homes and communities
• Assess patient and family needs, develop, implement, and support care plans, and maintain accurate healthcare records
• Support transitions with care team shifts including helping patients who are experiencing acute symptoms
• Perform nursing treatment including pain and symptom management
• As a RN, you can motivate and encourage a team of healthcare professionals in providing the best care possible to each patient
• Provide healthcare education to patients and family regarding pain and symptom management
• Monitor, record, and report symptoms or changes in patients' conditions in collaboration with Inter-Disciplinary team members
• Per Diem requirement-six weekend shifts 8am-5pm
