Job Summary :
We are seeking a compassionate Weekend Registered Nurse (RN) to join our team as a Case Manager in Wichita, Kansas. As a RN Case Manager with Specialized Home Care, you will work one-on-one with patients in their home environment to establish and oversee all aspects of case-managing patient's care. This role requires strong clinical skills, excellent communication, and the ability to work independently.
Responsibilities :
Assume responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care.
Collaborate with the patient, family / caregiver(s), ordering physicians, other care team members, and community resources.
Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
Participate in on-call rotation as reasonably assigned.
Qualifications :
Minimum of 1 year of nursing experience with a preference for 2+ years in an acute or post-acute care setting.
Active State Registered Nurse licensure is required.
Proof of current CPR certification and valid Driver's license.
