Develop Individual Service Plans (ISP) tailored to clients' specific needs, including housing, healthcare, behavioral health, employment, and social services. Provide ongoing case management, ensuring at least 50% of assessed seniors are actively engaged in support services. Conduct weekly check-ins with clients to track progress, address barriers, and adjust ISPs as needed. Facilitate placement into Long-Term Care (LTC) programs for seniors needing higher levels of support. Assist in roommate matching to promote affordable housing solutions for seniors at Gateway West. Provide 90 days of post-housing support to ensure long-term stability and connections to needed resources. Connect clients with primary care providers, behavioral health specialists, home health services, and chronic disease management resources. Coordinate access to nursing and therapy training and other essential healthcare needs. Identify and coordinate training for at least 10-15 interested seniors as Personal Care Assistants for the seniors (60+) at Gateway West. Connect certified PCAs with seniors (60+) at Gateway West, promoting housing opportunities while enhancing care availability for senior residents. Maintain accurate client records in the Homeless Management Information System (HMIS) and other required databases. Submit monthly and quarterly reports detailing service provision, client outcomes, and identified service gaps. Participate in networking activities and city-mandated evaluation programs to improve program implementation and outcomes. Assist in accessing public benefits, employment support, and financial assistance programs. Ensure culturally competent service provision, addressing systemic inequities in access to care and resources. JOB SUMMARY
The Care Services Coordinator (CSC) - City of Albuquerque Case Management role is designed to enhance the quality of life for seniors aged 60+ by providing comprehensive case management, housing stability services, healthcare access support, and long-term care coordination at Gateway West and in the broader community. The CSC will work to ensure seamless transitions for seniors experiencing homelessness into stable housing and supportive services, with a focus on long-term care placements, personal care assistant program facilitation, and resource coordination.
The successful candidate will be accountable for the full client care cycle, including outreach, assessments, individualized service planning, service coordination, follow-up, and quality improvement efforts. The CSC will work closely with city officials, community partners, and healthcare providers to ensure the delivery of holistic, client-centered care that aligns with the City of Albuquerque’s goals and the Aging and Adult Services (AAA) program requirements.
QUALIFICATIONS
- Experience in case management, social work, healthcare coordination, or related human services fields.
- Knowledge of homeless services, Medicaid billing, and long-term care programs.
- Strong interpersonal, advocacy, and problem-solving skills.
- Ability to maintain confidentiality and professionalism in all client interactions.
- Proficiency in data management systems (HMIS preferred).
- Bilingual (Spanish / English) preferred.
- Valid driver’s license and ability to travel locally as needed.
