JOB SUMMARY
The Care Services Coordinator (CSC) - City of Albuquerque Case Management role is designed to enhance the quality of life for seniors aged 60+ by providing comprehensive case management, housing stability services, healthcare access support, and long-term care coordination at Gateway West and in the broader community. The CSC will work to ensure seamless transitions for seniors experiencing homelessness into stable housing and supportive services, with a focus on long-term care placements, personal care assistant program facilitation, and resource coordination.
The successful candidate will be accountable for the full client care cycle, including outreach, assessments, individualized service planning, service coordination, follow-up, and quality improvement efforts. The CSC will work closely with city officials, community partners, and healthcare providers to ensure the delivery of holistic, client-centered care that aligns with the City of Albuquerque’s goals and the Aging and Adult Services (AAA) program requirements.
QUALIFICATIONS
- Experience in case management, social work, healthcare coordination, or related human services fields.
- Knowledge of homeless services, Medicaid billing, and long-term care programs.
- Strong interpersonal, advocacy, and problem-solving skills.
- Ability to maintain confidentiality and professionalism in all client interactions.
- Proficiency in data management systems (HMIS preferred).
- Bilingual (Spanish / English) preferred.
- Valid driver’s license and ability to travel locally as needed.
