Position Summary : Under the supervision of the Nurse Manager primarily assigned to River People Health Center (RPHC). The Wound Care Nurse specializes in assessing, treating, and managing patients with acute or chronic wounds. This role involves evaluating wounds, creating treatment plans, applying and changing dressings, and educating patients and families on wound care practices. The Wound Care Nurse collaborates closely with physicians, nurse practitioners, and other healthcare professionals to ensure the highest quality of care. This job class is treated as FLSA Exempt.

Essential Functions : Essential functions may vary among positions and may include the following tasks, knowledge, abilities, skills and other characteristics. This list of tasks is ILLUSTRATIVE ONLY and is not intended to be a comprehensive listing of tasks performed by all positions in this classification.

Examples of Tasks

1. Patient Assessment and Evaluation

  • Perform detailed wound assessments, including measuring and documenting wound size, depth, and tissue type.
  • Identify wound types and stages, including pressure ulcers, diabetic ulcers, and surgical wounds.
  • Develop individualized treatment plans based on patient history, wound characteristics, and risk factors.
  • Carries out medical provider orders.
  • Initiates appropriate interventions according to health care priorities identified in conjunction with primary care provider.
  • Administers medication as prescribed by physician or nurse practitioner.
  • Monitors medications and determines efficacy of medications, side effects and drug interactions.
  • Provides medication education to patients.
  • Assists with HHS employee health efforts including annual influenza vaccination clinic.
  • Participates as a member of multi-disciplinary consultation and review teams including case assignments.
  • RN will cross cover in the walk-in clinic which includes : RN triage by phone and in person triage, assessment of patients, medication administration, assist in managing in-baskets, and daily tasks.
  • Understand and be a part of the code blue / rapid response team.
  • Develops and documents nursing care plans and interdisciplinary treatment plans.
  • Establishes and maintains documentation and communication flow between staff at Salt River Clinic
  • 2. Wound Care Management

  • Clean, dress, and apply various types of wound care treatments, including debridement, negative pressure wound therapy, and specialized dressings.
  • Monitor wound healing progress, adjusting care plans as needed to optimize healing.
  • Manage wound-related pain and address any signs of infection or complications promptly.
  • Documents accurately, legibly and completely in Electronic Health Record (EHR), patient charts. Includes subjective and objective data, treatment provided, evaluation of treatment effectiveness and patient education provided.
  • 3. Patient Education and Support

  • Educate patients and their families on wound care procedures, prevention strategies, and signs of infection.
  • Provide resources on lifestyle adjustments, nutrition, and self-care techniques to support wound healing and prevent recurrence.
  • 4. Quality Improvement and Continuous Learning

  • Stay current with wound care advancements and best practices through continuing education.
  • Participate in quality improvement initiatives to enhance wound care services within the facility.
  • Educate and mentor other healthcare staff on wound care protocols and techniques.
  • Perform other job related duties as assigned to maintain and enhance departmental operations.
  • Knowledge, Skills, Abilities, and Other Characteristics :

  • Knowledge of the history, culture, laws, rules, customs and traditions of the SRPMIC.
  • Knowledge of the purpose, current issues, projects, organization, policies, and employee responsibilities of the division to which assigned.
  • Knowledge of medical, public health and social service resources available to Native Americans including, but not limited to, Indian Health Service, Medicare, Medicaid, ALTCS, SSI, etc.
  • Knowledge of public health practices and concerns.
  • Skill in documenting all wound care procedures, patient progress, and modifications to treatment plans accurately and in a timely manner.
  • Skill in wound assessment, infection control, and wound healing techniques.
  • Skill applying and interpreting federal, state, and agency laws and regulations governing professional nursing practices.
  • Ability to ensure compliance with healthcare regulations, standards, and best practices in wound care.
  • Ability to work as a member of a team.
  • Ability to assist with the coordination of all health related appointments and referrals.
  • Ability to coordinate health care aspect of patients if they transition to another hospital, treatment center or facility.
  • Ability to work closely with multidisciplinary teams, including physicians, physical therapists, and dietitians, to provide comprehensive care.
  • Ability to establish and maintain positive working relationships with colleagues, subordinates, supervisors, health care professionals, Tribal officials and representatives of resource agencies.
  • Ability to operate a variety of standard office equipment including cellular phones, photocopiers, FAX machines, calculators and personal computers.
  • Ability to incorporate cultural differences, health beliefs and learning styles.
  • Ability to identify and address programmatic problems.
  • Ability to collect, analyze, report and use data.
  • Ability to develop and implement program protocols.
  • Ability to maintain confidentiality and meet HIPPA requirements.
  • Minimum Qualifications

    Education and Experience : An Associate Degree in Nursing from an accredited college or university and certification as a Registered Nurse(RN) required. Bachelor Degree preferred

  • Active Arizona Registered Nurse license required.
  • Certification in Wound, Ostomy, and Continence Nursing (CWOCN) or similar certification required at the time of hire or within 2 years of employment.
  • Minimum of 2 years of clinical nursing experience, with experience in wound care strongly preferred.
  • Have Basic Life support certification
  • Experience with EPIC / OCHIN EHR system preferred
  • Experience working with Native American Communities preferred.
  • Experience working in a health clinic as a Registered Nurse preferred.
  • Equivalency : Any equivalent combination of education and experience that will allow the applicant to satisfactorily perform the duties of the job may be considered.
  • Underfill Eligibility : An enrolled Community Member whom closely qualifies for the minimum qualifications for a position may be considered for employment under SRPMIC Policy 2-19,
  • Special Requirements

    Special Requirements : May be required to work beyond normal business hours including nights, weekends and holidays.
