Bilingual Youth Case Manager

Glendale, AZ | Full-Time

Make a Real Difference in the Lives of Youth

At LivWell Behavioral, we don't just provide therapy — we show up for kids and families in ways that go far beyond a typical clinical setting. We're an outpatient behavioral health center in The Valley focused on growth-oriented group counseling and skill-building for children, and we're looking for a passionate Case Manager who's ready to be the love, the change, and the difference in a child's life.

If you lead with empathy, thrive in a collaborative environment, and want a career that feels like a calling, then LivWell is the place for you.

About LivWell Behavioral

Our mission is simple: Equip youth with the tools they need to live healthy lives. We deliver on that through interactive group counseling, individualized care, and community partnerships, including our collaboration with the East Valley Dream Center to provide families with food, essentials, and activities.

We hire around our core values: No Sidelines, Content not Control, People over Process, Self-Care before Community Care, and Praising the Failing. These aren't just words, they're how we work every day.

The Role

As a Case Manager, you'll be the connective tissue between our clients, their families, and the full spectrum of care they need. From intake through discharge, you'll ensure every child on your caseload receives individualized, coordinated, and compassionate support.

Schedule: Monday – Friday, 8:00 AM – 5:00 PM (some slight scheduling flexibility is required for specific cases)

What You'll Do

Intake, Assessment & Treatment Planning

  • Administer CALOCUS assessments to determine appropriate levels of care
  • Complete intake paperwork and conduct comprehensive mental health needs assessments
  • Develop individualized service, safety, and treatment plans for each client
  • Update treatment plans, risk assessments, and clinical documentation every six months or as needed
  • Develop and implement transition, discharge, and aftercare plans before services end

Service Coordination

  • Coordinate supportive services for clients and families
  • Communicate weekly with guardians and caregivers on treatment progress
  • Conduct at least one monthly in-person or virtual check-in with every family on your caseload
  • Participate in departmental staffing meetings and debrief with the BHT team

Client Advocacy

  • Advocate for client needs and ensure access to appropriate services
  • Assist families with tasks requiring extra guidance, such as AHCCCS appointments and crisis navigation

Facilitation & Community Engagement

  • Facilitate Child and Family Team (CFT) meetings
  • Attend quarterly school staff meetings to provide client progress updates
  • Build and maintain relationships with community resources and referral partners

Documentation & Compliance

  • Maintain accurate, up-to-date client records including case notes, service plans, and monthly progress reports in compliance with state regulations and company policy

Crisis Intervention

  • Provide crisis intervention and support to clients and families as needed

What You Bring

  • Spanish language proficiency strongly preferred
  • Strong organizational, planning, and task management skills
  • Ability to maintain composure under pressure and adapt quickly
  • Empathy and skill in building relationships with families from diverse backgrounds
  • Clear verbal and written communication with clients, families, and clinical professionals
  • Critical thinking and problem-solving ability
  • Comfort with technology, including Google Workspace and Adobe PDF
  • Ability to work both independently and collaboratively as part of a team

Education & Experience

  • Bachelor's degree in social work, psychology, or a related field preferred; relevant experience may substitute
  • Minimum of one year of experience (including internships or volunteer work) with children with mental health needs

Compensation & Benefits

  • Annual Salary: $40,000 – $45,000 per year
  • Health insurance
  • Dental insurance
  • Vision insurance
  • Life insurance
  • 401(k)
  • Paid time off
