Help Our Community's Transition Age Youth Gain Independence and Launch into Adulthood
Transitioning to adulthood is hard for anyone, but can be especially challenging for our most vulnerable current and former foster youth. TLC provides comprehensive mobile case management including employment, health and wellness, education, finance management, accessing resources, and navigating the ins-and-outs of independently living. We help youth (ages 16-25) to rise up from foster care successfully by providing a safe housing environment where they can learn and practice the life skills necessary to work on self-identified life goals.

This position requires dedication, commitment and patience in developing effective case management practices and plans while implementing policies and procedures in a teamwork model and harm reduction approach in accordance with agency policy and California Community Care Licensing Regulations, Title 22, Division 6.
