Pinnacle Home Care Floridas largest Medicare-certified home health provider has been delivering high-quality patient-centered care for over two decades and were looking for a Jr. Care Coordinator to join our award-winning team.

Key Responsibilities

• Conduct introductory welcome calls to patients providing a positive first experience and setting expectations for services.
• Verify physician signatures on orders and follow up as necessary to ensure timely completion.
• Maintain ongoing communication with patients regarding their status and care coordination needs.
• Collaborate with physicians to verify surgical protocols and required documentation prior to patient admission.
• Provide updates to physicians providers hospitals and other healthcare partners regarding patient status.
• Fax and email physician orders to obtain necessary signatures.
• Travel to hospitals physician offices or facilities as needed to secure signed orders (location-dependent).
• In certain cases travel to meet patients onsite at hospitals or facilities. (location-dependent).
• Escalate challenges or barriers to the branch location or leadership team in a timely manner.
• Work collaboratively across departments to ensure smooth patient transitions and resolve issues efficiently.
• Attend meetings to discuss patient status as well as challenges that need to be addressed.
• Manage and maintain an up-to-date patient tracking list including monitoring discharge dates orders and location changes.
• Ensure accuracy and organization of patient records and documentation across multiple computer systems.
• Support compliance with internal processes and healthcare regulations through accurate data management and communication.
