As a MAT (Medication Assisted Treatment) Care Coordinator, you will play a pivotal role in providing comprehensive support and care coordination for program participants in the MAT Program. Your responsibilities will include but not limited to connecting participants to necessary services and resources and monitoring their progress toward treatment goals. Under the supervision of the Utah Partners for Health Medial Director and Case Manager you will orient, enroll, educate, and coordinate Medication Assisted Treatment (MAT) services to eligible patients. As a member of the Care Team, the MAT Coordinator is also responsible for the planning and coordination of all physical, behavioral, and support services for MAT patients. The MAT Coordinator works closely with MAT providers in the delivery of MAT services to patients. This role requires excellent communication skills, a compassionate demeanor, and the ability to maintain composure in all situations.

ESSENTIAL JOB FUNCTIONS

• Assists in scheduling MAT patients for clinical visits, mental health visits, group visits and telehealth visits as appropriate and in a manner to maintain patient continuity.
• Schedule and arrange patient transportation, as needed.
• Functions as the hub in ensuring the MAT patient is actively engaged in counseling, as appropriate, while participating MAT and functions as a primary contract for addressing missed appointments and for MAT patients regarding buprenorphine refill requests.
• Provide patient support to those enrolled in the MAT program through follow-up calls and texts.
• Coordinate medication delivery and availability with internal and external pharmacies.
• Provide up-to-date resources to patients (housing, food, clothing, detox).
• Connect participants to necessary services and resources and monitoring their progress toward treatment goals.
• Attending meetings for MAT to update on progress.
• Coordinate with provider and nurse to address any concerns that patients may have.
• Administer MAT medications.
• Manage and follow-up on MAT and Behavioral Health referrals.
• Educate patients on MAT and how UPFH’s program works.
• Find and meet with partners in the community and strengthen relationships.
• Collect and track data for grant reporting.
• Prepare and transport supplies and equipment for Mobile MAT days.
• Manage Mobile Medical Clinic dates for MAT.
• Print off Controlled Substance Database reports for MAT providers.
• Continue to help expand the MAT program.
• Stay up to date on MAT CME’s where able.
• Provide patient summary emails for transition house residents such as Orange Street Clinic and Timpanogos Community Treatment Center.
• Ensure MAT contracts are completed by patients where necessary.
• Set up signed ROI’s where needed.
• Procure and sign-out Narcan.
• Ensure patient confidentiality.
• Collaborate with medical, behavioral health, and support staff to ensure a cohesive and integrated approach to participant care.
• Other duties as assigned.
