Job Title : Advanced Practice Clinician – Nurse Practitioner / Physician Assistant

Location : Fayetteville, NC (Travel between 2–3 facilities)

Duration : Approx. 3 months (Potential for hire)

Schedule :

Monday–Friday, 8 : 00 AM – 5 : 00 PM
No on-call, weekends, or holidays
40 hours per week

Program :

Senior Community Care (SCC) – ISNP

Overview :

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery model that coordinates clinical care for members in facility settings. As an Advanced Practice Clinician (APC), you will provide care to high-risk health plan members as part of an interdisciplinary team. This role offers work-life balance with flexible scheduling and no on-call requirements.

Primary Responsibilities :

Conduct comprehensive assessments

Manage acute and chronic medical and behavioral conditions

Ensure accurate documentation, including ICD-10 coding

Collaborate with interdisciplinary care teams

Lead advanced illness and care planning conversations

Provide counseling and education to patients and caregivers

Required Qualifications :

Active, unrestricted NP or PA license in state of residence (or ability to obtain)

NP : Graduate of accredited MSN or DNP program

PA : Graduate of accredited PA program

Board certification :

NP : AANP or ANCC (Family, Adult, Gerontology, Adult-Gerontology Acute Care)

PA : NCCPA

Current DEA licensure / prescriptive authority

Ability to meet physical requirements (lifting 30 lbs, navigating stairs, performing assessments)

Reliable transportation

Ability to obtain collaborative practice agreement if required

Full COVID-19 vaccination

Preferred Qualifications :

1+ year clinical experience (long-term care preferred)

Experience with complex behavioral, social, and functional needs

Knowledge of geriatrics, chronic illness, and end-of-life care

Proficient in EMR documentation
