Job Title : Registered Nurse (RN) Profession : Nursing Specialty : Home Health / Quality Assurance Duration : 13 weeks Shift : Day Shift Hours per Shift : 8 : 00 AM - 4 : 30 PM Experience : Minimum of 2 years Home Health experience, Previous quality experience License : Active Nursing License in Illinois Certifications : COS-C Certification (Certificate for OASIS Specialist-Clinical), CPR Must-Have : - Valid driver's license and car insurance - OASIS experience Description : Responsible for regulatory compliance, utilization review, chart audits, and performance improvement. Implements and evaluates performance improvement, quality, and utilization activities. Conducts reviews of medical records (plans of care, clinical and progress notes, etc.) for quality and compliance standards. Facilitates interdisciplinary case conferences, monitoring the coordination of care and OASIS data. Monitors OASIS data integrity and completes OASIS data submission. Ensures the development, implementation, and update of the individualized plan of care. Produces monthly reports to monitor the quality of services delivered and publicly reportable data. Assists with the evaluation of clinical competence and compliance. Remains informed and educated about home health rules, regulations, and standards. Ensures compliance with regulatory standards in the delivery of home health services. Promotes a clean and safe environment of care, utilizing error prevention habits. Provides the highest standard of privacy and confidentiality in matters involving patients and coworkers by abiding by the Standards of Conduct.
