Ohio Licensed Field RN Case Manager

A company is looking for a Field RN Case Manager.

Key Responsibilities

Manage patient care and coordinate services for assigned cases

Conduct assessments and develop care plans based on patient needs

Monitor patient progress and adjust care plans as necessary

Required Qualifications

Minimum of an associate degree in nursing

Active and unrestricted RN license in Ohio or a compact state

At least 2 years of clinical experience

Experience in case management or utilization review preferred
