Lifeline Connections is a community-based behavioral health organization that specializes in providing confidential and compassionate care to individuals who experience substance use and / or mental health conditions.

POSITION TITLE : Supportive Housing Specialist Non-Exempt Position

RESPONSIBLE TO : Program Director

MAJOR DUTIES : The Supportive Housing Specialist provides community based housing support services to individuals receiving treatment for behavioral health conditions and / or substance use disorders. This position provides clinical services to agency patients. In fulfilling these duties, the incumbent performs the following duties independently :

  • Provide community-based housing support services to individuals to meet the identified strengths-based housing goals including the housing search, placement and retention of permanent housing.
  • Establish and maintain relationships with community partners, landlords, and property management companies / owners.
  • Support individuals in filling out rental applications and other necessary housing documentation.
  • Assist individuals in obtaining housing financial assistance when needed, and requesting reasonable accommodations to overcome barriers to housing.
  • Collaborate with housing providers and landlords around any tenant issues; seeks to resolve concerns.
  • Coordinate services with program team and clinical staff to support individual's choice housing goals.
  • Maintain complete and up to date documentation of all services and contacts.
  • Represent the organization in a positive manner while working in the community.
  • Other duties as assigned.

JOB REQUIREMENTS :

  • A bachelor's degree in a related social services field with at least one year of paid or voluntary experience working in the coordination of Supportive Housing or in the coordination of independent living services in a social service setting
  • Experience providing housing support services preferred.
  • Two years' experience, including supervision, in the coordination of Supportive Housing or in the coordination of independent living services in a social service setting under qualified supervision.
  • Have an unencumbered driver's license, reliable vehicle transportation, and car insurance, and the ability to meet the agency requirements as an approved driver.
  • GUIDELINES

    This position also requires the incumbent to have a valid driver's license with a good driving record and a private vehicle in order to fulfill the duties of the position.

    COMPLEXITY

    The incumbent provides community based case management services to consumer / patients who differ widely in age and socioeconomic status and who may possess a variety of chronic and serious social, behavioral and psychological problems. Strong clinical skills, sound judgment, and creativity are required to help implement and deliver treatment services that meet the complex needs of the patients.

    PERSONAL CONTACTS

    Contacts are with the members of the treatment team, patients, significant others of the consumers, representatives of various community agencies specializing in the treatment of substance use disorder and mental illness, and related community representatives including medical personnel and landlords. All personal contacts are carried out in accordance with federal and state laws dealing with the confidentiality of patient records.

    PHYSICAL DEMANDS

    Most of the work can be performed while sitting and talking. The position requires the ability to travel within the community by private vehicle to meet the consumer / patient in their own environment. The position requires the ability to assist the consumers with daily life skills activities related to housing.

    WORKING ENVIRONMENT

    Working hours are spent indoors in offices or other meeting rooms or in the community meeting with consumers in their homes. Supervision of outdoor activities, visits to community agencies, and participation in staff retreats and staff development activities will be required.

    IMMEDIATE SUPERVISOR : Program Director

    Salary : $20.39 - 23.21 / hr DOE
