What You'll Do :

• Evaluate and treat patients with communication, speech, language, cognitive, and swallowing disorders
• Deliver personalized therapy plans that promote patient independence and well-being
• Collaborate with interdisciplinary teams to coordinate care and support discharge planning
• Engage patients and caregivers to optimize outcomes and improve quality of life

What We're Looking For :

Epic EMR experience preferred
