JOB DESCRIPTION :

The Case Managers are accountable to the Quality Supervisors as well as the Chief Administrator for working with an Administrative Team in the planning and oversight of assigned aspects of the youth residential experience. Case managers will be responsible for assessing each youth in a CANS and FANS assessment and putting the services in place in order for the youth to be successful. Case managers will also be responsible for working in correlation with youth family members, MDHHS workers, and other community agencies. They will facilitate Family Team Meetings and will attend all IEP meetings, school enrollment and orientation, school conferences, court hearings, adoption meetings and any other meetings that support the youth and their transition. Case managers will work with local community resources to put services in place that best serve each youth and their direct needs. Case managers will assist with identifying mentors and support people for their youth.

Case managers will participate in the development of initial service plans within 30 calendar days of the youth placement with LTAI and quarterly thereafter. They will work in correlation with the Independent Living skills trainer, Chief Administrator, Case Manager Supervisor, and other members of the LTAI administrative team to ensure that youth (and potential family members) are prepared for a transition from LTAI. Case managers will provide case management service and aftercare services to discharged youth for up to one calendar year.

MAJOR DUTIES & RESPONSIBILITIES :

• Responsible for verbally de-escalating and resolving conflict, aggression, & anger, etc.
• Enforce all LTAI & ACTS II policies.
• Complete appropriate paperwork during your shift (i.e. incident reports, one-on-one reports, electronic communication log, etc.).
• Must be able to sympathize with resident’s needs and feelings regardless of resident’s race, social, ethnic, and religious background
• Case Managers must attend all monthly staff meetings and training.

This job operates in a home-like setting working with youth ages 10-17. The position will require consistent interaction, focused engagement, ability to adapt in a fast paced environment, and daily supervision of youth.

POSITION TYPE / EXPECTED HOURS OF WORK :

Our facility is open and staffed 24 / 7, 365 days a year. The position is part time or full-time Monday through Sunday. Extended hours may be necessary as needed to assist and / or cover staffing during program hours.

REQUIRED EDUCATION & EXPERIENCE :

B.A. or B.S. Degree in Social Work or related field from an accredited college with preferable experience in a Child Caring Institution, Child Placing Agency, AFC home, or related human services field.

ADDITIONAL ELIGIBILITY QUALIFICATIONS :

1. Must be able to successfully pass fingerprinting, abuse / neglect registry, pre-employment drug test and physical, driving record clearance process, and criminal background check.

2. Have a strong working knowledge of computer systems, including Microsoft Office Suite.

3. Follow HIPAA guidelines and maintain confidentiality.

4. Must have a Michigan valid driver’s license.

5. Reliable transportation and telephone.

LTAI C.M.H.S. Inc. is a licensed and contracted child caring institution providing open residential foster care treatment services to adolescent boys and girls ages 10 to 17. We offer many valuable services to assist the needs of our residents and to teach them the skills they need to transition to a parent / guardian, a foster home, AFC home, to an independent living or to a supported independent living setting. Many of the youth that we serve have suffered severe trauma through physical, sexual, emotional , and abuse / neglect. Some of the residents may also have developmental disabilities and / or cognitive impairments where they may struggle with toileting, hygiene, physical activity, language functioning, emotional regulation, and learning disabilities. These youth may also struggle with over sexualized behaviors.
