We're looking for a mission-driven Advanced Practice Provider (APRN or PA) to join our team of dedicated professionals working in an integrated primary care and behavioral health setting. Responsibilities Provide comprehensive primary care services including acute, preventive, and chronic disease management for pediatric through geriatric patients. Conduct health assessments, physical exams, and develop treatment plans in collaboration with a multi-disciplinary team. Serve as a prescriber in accordance with Illinois law and agency policy; participate in collaborative physician meetings and case discussions. Contribute to integrated care by participating in team huddles, staffing meetings, and consultation activities. Use electronic health records (EHR) for documentation and meet expected productivity, quality, and documentation standards. Assist in maintaining compliance with HRSA, Joint Commission, and other regulatory / accrediting bodies. Support continuous quality improvement and program development efforts. Qualifications Master's degree in Nursing with RN licensure and certification as a Family Nurse Practitioner (FNP) or degree from an accredited Physician Assistant program. Licensed in the state of Illinois as an APRN or PA. Experience in primary care, community health, or ambulatory settings preferred. Strong knowledge of chronic disease management and preventive care. Commitment to serving under-resourced and diverse populations. Proficient in basic computer skills with willingness to use an electronic health record system. The anticipated starting pay for new hires for this position is between $96,250 - $110,000 annual salary.
