EMR :

HCHB (Homecare HomeBase) experience required

Experience :

Minimum of 1 year of recent Home Health experience

Productivity :

6 daily visits or a minimum of 30 points weekly

Average Patient Load :

5–7 patients per day (to be confirmed during interview)

Case Type :

Primarily geriatric patients

First-Time Travelers :

Not accepted

Certifications Required :

BLS

Licensure :

Active license required prior to start

Special Requirements :

Flexibility with assigned territory

Timely completion of documentation and consistent communication
