Role Snapshot

Transform bedside care with AI. As Circadia's on-site NP, you'll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions-reducing preventable readmissions. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.

Must be willing to commute to Bronx and Brooklyn.

What You'll Do

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech - bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows, and run live demos during real patient encounters.
• Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: Document outcomes, refine protocols, and lead brief huddles to sustain adoption and measurable impact.
• High-volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows, summaries, and flagging logic; participate in root-cause reviews.
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover nearby sites (30-40-mile radius) as needed; mileage and lodging covered per policy.

How Success is Measured (KPIs)

• Visits: Target 25 patient visits/day (or 500/month, census-dependent).
• Engagement: ≥80%
• Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Qualifications

• Certified and licensed Nurse Practitioner in New York; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid New York driver's license; comfortable traveling locally.
• Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Thrives in a dynamic, evolving startup environment.
• English required; Spanish preferred.

$160,000 - $200,000 a year
