The CNA will support Vitra's Adult Day Health (ADH) center in Brockton, MA. This is a daily care facility designed to meet the physical, functional, and social needs of seniors or individuals with disabilities, including those with complex medical or rehabilitative needs.

Essential Functions :
Our Adult Day Health CNA position provides care under the supervision of a Registered Nurse - RNCNA assist the Adult Day Health participants with Activities of Daily Living (ADLs) including skilled tasks when applicableActivities of Daily Living are personal care focused and may include assisting clients with bathing, grooming, dressing, ambulation, eating, and toiletingAssist members in the practice of safety and good nutrition when possibleParticipate in assessment and care planning meetings and execute the plan of careAssist Registered Nurse with obtaining and recording vital signsAdhere to the documentation and care procedures and standards of personal and professional conductObserve the patient for changes in general physical and mental conditions and report any changes noted to the Adult Day Health program Registered NurseAssist with all the activities programs including arts and crafts and other small group activities under the direction of the DirectorAccurately documents care provided daily and maintains records per regulatory standards.Recognizes the physical, emotional and developmental needs of participants, working in a manner that respects participants, their privacy and their propertyAssist with trips and other community outings outside the programParticipate in staff meetings, in-service training, and other relevant trainingPrepare, serve, and clean up after meals and snacksOther duties as assignedQualifications :
Active Massachusetts' CNA certificationPrevious experience in nursing home strongly preferred.Ability to read and follow written instructions and documents.Has empathy for the needs of the clients.CPR certification or willing to obtain.One or more years of experience working with adults in a healthcare or social service setting.Physical and Environmental Demands :
