Job Details

  • Performs patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals.
  • Develops and revises plan of care for patient and family based on evidence-based knowledge.
  • Maintains visit productivity standards and demonstrates knowledge of insurance reimbursement.
  • Completes admission, discharge, and re-visit assessments and documentation within required timeframes.
  • Uses age-appropriate teaching methods for patients and families and documents education tools used.
  • Assumes responsibility for case management including prioritizing care needs and interdisciplinary communication.
  • Performs HHA supervisions per standards and follows scheduling process.
  • Maintains safe environment by following infection control measures and reporting infections.
  • Demonstrates knowledge of pharmacology and safe medication management, including medication reconciliation.
  • Participates in staff meetings, performance improvement programs, and utilization review meetings.
  • Provides orientation and mentorship to new employees and cross-trains to learn new skills.
  • Assists with growing agency business and accepts other assignments as requested.

    • Job Requirements

  • Must have 2-3 years as RN in home hospice environment.
  • Current New Jersey RN License.
  • Experience using an EMR.
  • Excellent critical thinking and communication skills.

  • Contract length is 13 weeks.
