Job Details

Performs patient assessments to determine appropriateness for hospice care and develops care plans for patients and families.

Ensures appropriate utilization of services and maintains visit productivity standards.

Completes discharge assessments and documentation within required timeframes.

Uses age-appropriate teaching methods for patients and families and documents education tools used.

Assumes responsibility for case management including developing, implementing, and evaluating plans of care based on evidence-based knowledge.

Demonstrates knowledge of hospice regulations, insurance reimbursement guidelines, and interdisciplinary communication.

Maintains a safe environment by following infection control measures and safe medication management practices.

Participates in staff meetings, performance improvement programs, and provides orientation and mentorship to new employees.

Assists with agency business growth and cross-trains to learn new skills.

Job Requirements

Must have 2-3 years as RN in home hospice environment.

Must have valid New Jersey driver's license.

Current New Jersey RN License.

Experience using an EMR.

Excellent critical thinking and communication skills.
