• Previous discharge planning and acute care case management experience
• Experience meeting with families and discussing plans of care

Description :

Case Managers provide care for patients in all acute care departments.

The RN Case Manager will most likely remain in the same area during their contract, pending new hires.

Typically, travelers are not required to float to multiple areas during their contract.

The patient ratio is 1 Case Manager to 16-20 patients.

Support on the unit includes Case Management Assistants, Lead Case Managers, Social Work Complex Psychosocial patient consultants, and Utilization Review RNs.

The charting system used is EPIC.

Scheduling includes a weekend rotation of 2 weekends per 13-week assignment.

Typical staffing support during weekend shifts consists of 3 RN Case Managers, 1 Social Worker, 1 Lead Case Manager, 1 Case Manager Assistant, and 1 Utilization Review Nurse.

On-call duties are not required.

Holiday expectations include working 2 out of 4 major holidays.

Time off approval allows for up to 7 days off, must extend for any full week off.

Scrub color is navy blue or professional attire.
