LPN Licensed Medical Coordinator

A company is looking for a Medical Care Coordinator.

Key Responsibilities

Coordinate outreach to and respond to members regarding medical conditions, benefits, and coverage criteria

Assist members in navigating complex healthcare systems and guide them through medical resources and policies

Work with members identified with rising health risks to provide education and reinforce treatment plans

Required Qualifications

Associate's degree with a healthcare focus or LPN certification

Background in Social Work or as a Registered Nurse
