RN Triage Care Manager

Are you a compassionate RN with a passion for primary care and care coordination? A well-respected rural healthcare organization in Orleans County, New York is seeking a RN Triage Care Manager to join its dedicated team and make a real impact on the lives of patients in the community.

In this role, you ll work closely with a diverse patient population, providing triage, care coordination, and support to those most at risk. You ll be an integral part of a supportive environment where your clinical expertise and leadership truly matter.

What You ll Need :

• Active RN license with 3 5 years of relevant experience
• Background in any of the following : triage, case management, disease management, home health care, nursing, hospital nursing or intensive outpatient education and / or self-management support
• CPR / BLS Certification
• EMR and Microsoft Office proficiency
• Excellent communication and clinical decision-making skills
