• About Facility :

o Home Health Services - serving the following areas : Cleveland, Gaston, Rutherford Counties

o Radius rule : Traveler's permanent address must be 50 miles outside of home unit

o Typical Caseload :

o Minimum Years of Experience required : 1 year - Home Health experience

o Will you accept a first-time traveler? Yes - if they have had Home Health experience as a nurse

o 18 y / o + (no peds), wd care, wd vacs, Pleurx drains, cath (foley and SP), feeding tubes, IVs (peripheral and PICCs / Ports), DM, HF, med and disease mgmt.

o Teammates will travel throughout the below areas :

o Driver's License and proof of Insurance

o BLS

o Oasis, hospice philosophy, infection control practices, wound vacs, wound care, PICC line dressing changes, port access, IV infusions, drains, critical thinking skills, catheter insertion, drains

o Ability to drive at night

o Leadership

o LPN / CNA

o Peers

o Case Management

o EMR : HCHB (Homecare Homebase)

o Samsung note 9 phones

o EPIC Rover - view only

o Focura - will upload discharge instructions and other important docs such as wound pictures

o NEO

o 2 days departmental

o RTO : Manager approval
