- Certified home health experience including proficiency with OASIS (non-negotiable).
- Ability to work independently with minimal supervision / assistance.
Description :
COVID vaccination (dose date on or after 10 / 3 / 2023) is now required.
Exemption forms are available for those who qualify.
If approved, travelers will wear a mask at all times while on site.

Candidates must not reside within 50 miles of the facility.
Requested time off during this assignment must be noted and verified with profile upload.
No more than 5 days of requested time off will be accepted.

This is a home health position.
The physical therapist is responsible for providing physical therapy services to clients.
Primary functions include direct patient care, collaboration with team members, and participation in developing the patient's comprehensive plan of care.
Rehabilitation equipment includes splints, orthoses, prostheses, wheelchairs, and ADL equipment modalities.
