LPN Intake Coordinator

Summary

We are hiring for a Full-Time Home Health LPN Intake Coordinator to join our passionate team in Wallingford, CT!

• This is an in-office position Monday-Friday, 8 : 30am- 4 : 30pm.

Pay range : $35 -$39 hourly, depending on experience

Responsibilities

The Intake Coordinator receives and analyzes incoming referral and order calls while effectively communicating patient / referral information to appropriate teams.
