Job description
A company is looking for a Nurse Case Manager - Maternity (must be bilingual in Haitian Creole).
Key Responsibilities
Assess members' needs and develop individualized care plans
Monitor care plans and coordinate with providers to ensure service delivery
Provide education and referrals to community resources for members
Required Qualifications
Bilingual in Haitian Creole
Registered Nurse (RN) with a current, unrestricted compact license
Preferred college degree
Three or more years of experience in a Case Management role
CCM certification required within specified timeframe
