Responsibilities :

Conduct initial intake interviews

Complete assessments and identify barriers

Develop and implement individualized goal-oriented treatment plans

Interface with various organizations

Coordinate and link clients to appropriate care and community resources

Document services provided to clients by completing progress reports

Assist clients with finding permanent housing solutions

Conduct follow-up

Provide education

Facilitate groups

Date entry

Attend meetings and training

Utilize zoom for telehealth appoints and trainings

Qualifications :

1-year previous experience in counseling, social work, or other related fields

Ability to read, write, and understand English

Computer Literate a must

Associates Degree a +
