Overview

Easterseals MORC is hiring for a Case Management Coordinator – Children's Intensive Services to help make a difference and become part of something bigger than yourself.

Responsibilities

  • Performs crisis intervention as needed.
  • Demonstrates the ability to engage individuals in a welcoming, hopeful, empathic manner regardless of disability or phase of treatment.
  • Assesses and evaluates the needs of individuals and maintains caseload contact to develop goals. Identifies stage of change and uses interventions consistent with stage of treatment. Assists clients in developing strength-based, stage-wise, person-centered plans designed to address identified needs. Completes referral forms, clinical and legal documentation to obtain community-based services, and collaborates with other SA / MH community partners.
  • Advocates and provides options for supports and services as needed; develops community resources to meet the needs of assigned individuals for activities and support.
  • Assists individuals with identifying changes in behavior, attitude, feelings and thoughts that are triggers; assists in developing relapse prevention plans and crisis plans utilizing a variety of supports including personal, community and treatment interventions.

Qualifications

  • Bachelor's degree in a Human Service Field from an accredited school, with three years supervised experience in the examination, evaluation, and treatment of minors and their families.
  • Must be a CMHP in accordance with Medicaid Provider Manual Guidelines.
  • Must possess a valid Michigan motor vehicle operator's or chauffeur's license.
