Firefly Children & Family Alliance is looking for a full-time Home-based Case Manager in Muncie, Indiana. You will serve Grant and Delaware county. Case Managers provide services to families in the families' homes on a routine basis to assist them in obtaining their goals. You will need to provide education and support to these families and transportation. You will support your clients by attending court, responding to crisis and stabilizing family functioning. This position is field-based and will primarily work out in the community, with some evenings required.
You Will :
- (Family Progress - Monitoring and Coaching (50%)
Coach collaboratively - maintaining a balance between teaching and serving
Visit each youth / family on a routine and 'pop in' scheduleAssist youth / families in achieving their goalsProvide education and support to youth / familiesProvide transportation to youth / familiesAttend court with clients and render recommendationsCommunicate with schools and multiple institutions related to youth / family mattersRespond to all crisis calls and manage those situationsService Plans / Goal Development (20%)
Create plans, including goals, for each youth / familyUpdate plans and goalsDevelop relationships with agencies in the communityWork with other social welfare agenciesIntake and Needs Assessment (15%)
Determine 'crisis' situationsVisit each youth / family to gather necessary information to complete the assessmentFollow Firefly Children & Family Alliance professional standardsReport (15%)
Maintain case and agency paperwork according to standardsPrepare written reports for the Department of Child Services / Probation office within identified timeframesYou Have :
Bachelor's degree in social work or related field is required.Must be at least 21 years of ageA valid driver's license and insurance with no more than 6 points on driving recordTravel between appointments to service clients within their communityKnowledgeable regarding social service systems within the stateKnowledge of and adherence to current confidentiality practicesAppreciation for and ability to respond to cultural differencesDevelop relevant networks of contactsOur Benefits :
