Ohio Licensed RN Care Manager

A company is looking for a Remote Registered Nurse (RN) Care Manager - Home Health.

Key Responsibilities

Develop and oversee the patient plan of care, ensuring regulatory compliance

Participate in interdisciplinary team meetings and manage healthcare costs through effective care planning

Review charts for compliance and function as a resource for clinicians and agency staff

Required Qualifications

Registered Nurse licensure required

Two years of home care field experience preferred

One year of clinical review experience preferred

Two years of case management and / or utilization management experience preferred

Proficiency in HCHB preferred
