StartDate : 4 / 21 / 2025 Available Shifts : 8 D Pay Rate : $1819.54 - $1993.75

Illinois licensed PT for Home Health duties in New Lenox. 8a-4 : 30p Mon-Fri with 40 hours / week guaranteed. BLS, 1 year Home Health expereience required. Covid vax (no declinations) and valid drivers license / insurance required.

Years of Experience : 1. Illinois state license; BLS

1 year Home Health PT experience - REQUIRED

Valid drivers license and car insurance - REQUIRED
