Summary he Community Living Center (CLC) is comprised of 120 beds among three neighborhoods : CLC North, CLC South, and Hospice. Services provided to Veterans of all ages include Short-Stay Rehabilitation, Short-Stay Skilled Nursing Care, Short-Stay Continuing Care, Long-Stay Continuing Care, Respite, and Hospice and Palliative Care. The mission of the CLC is to restore each Veteran to his or her highest level for well-being, prevent declines in health, and provide comfort at the end of life. Responsibilities Total Rewards of a Allied Health Professional The LPN is part of the nursing team and is committed to providing resident-centered care from a holistic perspective focusing on the resident's physical, psychological, social, and spiritual wellbeing. The LPN participates in resident care planning and serves as a member of the nursing team. The LPN performs assigned duties in a manner that promotes quality health care, positive customer service, and the maintenance of an environment committed to serving the Veteran. Duties include but are not limited to : 1. Respond to resident calls for assistance and address resident needs in timely manner. 2. Ensure proper identification of resident per policy before administering care. 3. Willing to assist co-workers without being asked and work well as team. 4. Actively participate in change of shift, huddles, and round. Communicating information to Team Leaders, Charge Nurse, team members, and / or receiving shift. 5. Assist RN with admissions, transfers, and discharges of residents. Completing all duties as assigned and under the supervision of the RN. 6. Give bed bath / shower, trim nails and assist residents with grooming, He / she will consistently promote resident independence, as the resident plan of care dictates. 7. Provide and assist with oral hygiene and denture care. 8. Change linen of occupied and unoccupied beds. 9. Assist residents with toileting, beds pans, urinals, etc. 10. Feed residents, assist those who need help with eating. Adhere to aspiration precautions. 11. Change resident's position, maintain body alignment and take measures to prevent skin breakdown and / or deformity. 12. Provide postmortem care. 13. Practice safety precautions according to written procedure in performance of duties when working with residents and resident's personal care devices, medication administration, and unit equipment. 14. Perform duties in a safe manner to prevent resident / employee accident / injury. LPNs at this grade level are expected to perform all duties within the scope of their license (e.g., medication administration); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved. GS-04 Employees may be appointed at or advanced to GS-4 if they meet any of the criteria below : Six months of qualifying experience as an LPN or LVN; or Graduation from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; Graduation from an approved school of at least 24 months duration. Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and / or behavioral problems. Works with the RN and / or MD / DO to appropriately orient / train less experienced LPNs / LVNs and / or Nursing Assistants (NAs) / Health Technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and / or family members in relation to common disease processes, medication, and / or prescribed treatment regimes. Recognizes and appropriately responds to breakage / malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action. GS-06 Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN / LVN. Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations. Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant] patient information. Physical Requirements : This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, kneeling, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative / invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
