Job description :

  • Setting : Skilled Nursing Facility, Sub-acute and long-term care patients
  • Schedule : Minimum of (2) days on site per week. Telehealth support is available on-site for virtual interactions
  • Must be licensed in VT
  • Experienced in SNF preferred, not required
  • Potential : 200-300K (Base Salary + RVU Bonus Model)
