We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our expanding care team. This individual will play a key role in supporting patients’ health journeys and combining clinical expertise with integrative wellness practices to deliver high-quality, patient-centered care.

Key Responsibilities

• Conduct virtual and / or in-person patient consultations, assessments, and follow-up care.
• Develop and implement comprehensive treatment and wellness plans tailored to patient needs.
• Collaborate with mental health professionals, case managers, and primary care physicians to ensure coordinated care.
• Provide post-discharge support to patients transitioning from hospital to home environments.
• Educate patients on preventive care, medication management, and holistic health strategies.
• Maintain accurate electronic health records in compliance with HIPAA standards.
• Participate in DesireMD’s wellness programs, including stress management, mindfulness, and lifestyle education initiatives.
• Contribute to continuous improvement of DesireMD’s telehealth and integrated wellness programs.
