A company is looking for a Nurse Practitioner - West Coast.

Key Responsibilities

Oversee monitoring of patient vitals, symptoms, and labs in collaboration with the healthcare team

Manage treatment plans and medications, focusing on guideline-directed medical therapy for chronic conditions

Conduct regular virtual check-ins with patients to review labs and optimize treatment adherence

Required Qualifications

5+ years of experience treating patients with chronic diseases (T2D, Hypertension, CHF)

Active multi-state compact RN license and NP licenses in CA and WA

ANCC or AANP certification required

Master's Degree as a Nurse Practitioner

Experience managing CHF with guideline-directed medical therapy
