Definition
Responsibilities include initial interview and assessment of individuals seeking services through the Pathway to Care and Recovery (Engagement Center) and Safe Place to Stay to ensure individual is medically stabilized and fit for care. This individual will interact regularly with clients who are actively intoxicated and / or in varying stages substance-use recovery. The LPN, Engagement Center will conduct medical triaging of newly arriving clients to ensure the individual is medically stabilized and fit for care and monitor current clients with the Pathway to Care and Recovery for potential withdrawal symptoms and / or complications. Utilizing the information collected from newly arriving clients during triaging, this individual will provide input into decision-making in regard to client admission to the Safe Place to Stay and / or other external referral sources such as external detoxification providers, external substance use disorder treatment providers, external MAT providers and external medical facilities.
Eligibility Requirements
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and / or ability required. This individual must be available to work a part-time daylight hour schedule.
Education and / or Experience
LPN licensure required. Medical and behavioral healthcare experience preferred. Valid PA driver’s license and Act 33 and 34 clearance required. Must be organized and detail orientated.
