Department: Med Surg 5E
Shift Timings: 18:45-07:15 Hours Monday to Friday (3 Days/12 Hours)
Per Week: 36 Hours

Unit Capacity: 34 Beds Population Served: Adult and geriatric medical/surgical patients. Ages of Patients Served: Young Adult through Late Adult (18 - 60+) Nurse to Patient Ratios: 1:6 Core Staffing Plan: Variable staffing grid to include: Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Patient Care Technicians (PCTs), Nurse Interns (NIs). An RN assumes full responsibility for the patients on each team. Staffing Certification: Basic Life Support (BLS) Certification Successful completion of a rhythm test

Roles & Responsibilities
Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
Assess and coordinate patient's discharge planning needs with members of the healthcare team.
