North Star Health, an independent Federally Qualified Health Center comprised of multiple PCMH practices delivering team-based care in Southern VT and Southwestern New Hampshire, is seeking an experienced nursing professional who is passionate about community-based care to join our leadership team as Chief Nursing Officer. The CNO will set standards for nursing practice, support and develop all levels of staff in models of integrated, team-based care, lead our community health and care coordination teams, and strengthen our efforts to achieve the Quadruple Aim and our Mission and Vision “to connect with and care for our community so everyone may enjoy health and wellness.”

In this Role You will :
- Play a leading role in the recruitment, development and support of a dynamic and multi-level clinical support staff; support the efficient deployment of clinical support staff, ensuring the right skill mix to promote the highest level of care
- Develop a positive culture to promote continuous growth and staff development, establish best practices and the education and support systems to sustain them
- With executive leadership, medical and nursing staff and key managers, participate in the development of health care policies, procedures and protocols while ensuring their consistency with applicable laws, regulations, program expectations, and professional standards.
- Develop and implement care coordination in accordance with FQHC, PCMH, Blueprint for Health and ACO standards, values and models
- Provide supervision and mentorship to Medical Assistants, LPNS and RNs in our health centers supporting responsibilities including but not limited to triage, rooming, medication prescription refill management, Care Coordination, immunizations, infection control, employee health, preventative health education and promotion
- Serve as the Community Health Team Leader, providing supervision and mentorship to CHT staff,[LT1] [LT2] including RN care managers / coordinators, CHWs, CDEs and RDs, and MOUD RN staff
- Be a champion for quality improvement and innovation, encourage and lead QI projects, evaluate workflows, develop innovations and participate in data reviews and learning collaboratives
- Establish and maintain relationships with community service providers; serve as a resource to staff on how to coordinate client access to services.
- Participate in senior-level decision-making, strategic planning, the development of business initiatives, and advance of the overall mission and goals of our organization.
- Develop and promote partnerships with providers, payers, community organizations, the ACO, employers and other stakeholders to achieve quality and population health strategic goals and programs.
Compensation details : 120000-140000 Yearly Salary
