What You’ll Do :

As our new Occupational Therapist , you’ll be at the heart of our care team, delivering personalized therapy services that empower patients to live more comfortably and confidently. Your responsibilities will include :

• Providing direct therapy treatments in patients’ homes
• Assessing home environments and developing personalized care plans
• Collaborating with physicians and other healthcare professionals
• Monitoring and reporting patient progress and adapting plans as needed
• Identifying additional care needs and coordinating with home health services
• Supervising OT assistants in accordance with policy and regulations
• Supporting patients and families through discharge planning and beyond
