• Must have completed 50 deliveries and 25 GYN Cases in the last 12 months to be considered as this is a requirement for credentialing
• Need to have a full MI license, DEA, and Cs3 or IMLC
• Medical School on California approved list
• Residency from ACGME Approved list
• No large malpractice issues
• Clean NPDB
