Speech-Language Pathologist (SLP) - Skilled Nursing Facility | Waverly, NY | Full-Time | CFY Welcome | Sign-On Bonus

Key Responsibilities

Conduct comprehensive speech-language evaluations and assessments for skilled nursing residents
Develop and implement individualized treatment plans for speech, language, voice, cognitive-communication, and swallowing disorders
Provide evidence-based speech therapy interventions to improve communication and dysphagia management
Collaborate with interdisciplinary team members including physicians, nurses, occupational therapists, and physical therapists
Document patient evaluations, treatment sessions, progress notes, and discharge summaries in compliance with regulatory guidelines
Participate in care planning meetings and communicate treatment progress to families and caregivers
Perform audiometric screenings and coordinate appropriate referrals for hearing services
Provide staff education and training on communication strategies and safe swallowing techniques
Maintain compliance with state and federal regulations, facility policies, and safety protocols
Contribute to quality improvement initiatives and resident-centered care programs

Required Qualifications

Master of Arts (M.A.) or Master of Science (M.S.) degree in Speech-Language Pathology from an accredited program

Current New York State license to practice as a Speech-Language Pathologist (or eligibility to obtain licensure)

ASHA Certificate of Clinical Competence (CCC-SLP) preferred; Clinical Fellowship Year (CFY) candidates enthusiastically welcomed

Strong clinical assessment and treatment planning skills

Excellent written and verbal communication abilities

Proven ability to work effectively within a multidisciplinary healthcare team

Commitment to delivering high-quality, patient-centered care in a geriatric setting

Proficiency with electronic medical record (EMR) documentation systems

Compassionate, professional demeanor with strong interpersonal skills
