Supervisor Responsibilities : No

Location : 121 W. 154 th St in Harvey, IL

Purpose : The Activity Aide implements recreational and activity services in compliance with CHC policies and procedures .

Responsibilities :

• Implements activity and after school programs according to the written schedule
• Document progress and attendance for all activities (both group and individual)
• Completes documentation daily
• Assists with completion and implementation of documentation of habilitation plans
• Maintains records and prepares requested reports
• Complete activity notes by the 10 th of each month
• Participates in the continuing interdisciplinary evaluation of individual residents needs
• Assures there is written permission given by the resident's physician to participate in the activity program
• Implements the activities to coordinate with other departments to e nsure communication between all departments to provide uniformity of goals
• Participates in transporting students to and from the activities
• Assist in the development and implementation of special events
• Assist in the coordination of holiday visits from various groups / foundations
• Suggest ideas for in-house and community activities
• Attends seminars / workshops / facility / department meetings as necessary
