Position Summary :

We are seeking a dedicated and experienced Director of Nursing (DON) to lead the clinical operations of our growing home health agency in Indiana. The ideal candidate will have strong leadership skills, a deep understanding of OASIS documentation, and experience in compliance with CMS regulations and Indiana state requirements.

Key Responsibilities :

• Oversee day-to-day clinical operations and supervise all field and office clinical staff.
• Ensure timely and accurate completion and submission of OASIS assessments, including SOC, ROC, Recertification, and Discharge.
• Maintain compliance with federal, state (Indiana Department of Health), and accreditation standards.
• Participate in the development and implementation of policies and procedures.
• Conduct performance evaluations, training, and in-services for nursing and paraprofessional staff.
• Review clinical documentation for accuracy, completeness, and compliance with Medicare and Medicaid regulations.
• Cooperate with physicians, case managers, and other healthcare professionals to coordinate patient care.
• Participate in quality assurance / performance improvement (QAPI) initiatives.
• Serve as a clinical resource for staff and assist in complex clinical decision-making.
• Manage staff schedules to ensure appropriate coverage and efficient service delivery
