We are searching for a talented Advanced Practice Practitioner to join our Care Navigation Team. / Mobile Integrated Health program.

Hours : Full-time - 40 hours M-F 8A-4 : 30P

Shift : Day - Standard - no weekend, holiday or on-call.

Location : Fairhaven, MA
