This will include completing a comprehensive psychiatric / medical assessment and recommendations on plan of care to patients medical provider. • Provide home-based care to patients with illnesses, injuries and chronic diseases-often after discharge from a hospital, rehab or nursing facility\n
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care\n
• Examine patients, do assessments, document progress and report changes to their physician\n
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise\n
• Coach family members on the patient's plan of care, medication and home safety\n

• Registered Nurse (RN) with active, unrestricted license in the State of Michigan - BSN not required\n
• At least one (1) year of clinical experience as a Registered Nurse\n
• At least one (1) year of psychiatric experience\n
• Current CPR Certification\n
