Arizona Licensed Behavioral Health Clinician

A company is looking for an AZ Licensed Clinician : LCSW in Behavioral Health and Family Medicine.

Key Responsibilities

Provide customized care and treatment plans for patients in assigned Care Pods

Coordinate behavioral health care with primary care providers and monitor patient symptoms

Document patient progress and treatment recommendations in Salesforce and electronic health records

Required Qualifications and Education

Master's Degree in Social Work, Counseling, or a related field

Licensed to practice in Arizona as an independent clinician (LCSW / LICSW, LMHC, LMFT, LPC, or equivalent)

Ability to complete cross-licensing in Montana if required

Experience with evidence-based treatment approaches is highly preferred

Fluency in English; proficiency in Spanish is highly preferred
